  • Mother’s maiden name

  • Date of birth

  • Social security number

  • Debit card number

  • Expiration date & security code

  • Billing address

  • Physical address

  • Home of record (if different from above)

  • DOD ID number

  • CAC PIN

  • SIPR token PIN

  • Your current unit

  • IP address for your work computer

  • Locations you’ve been deployed

Also, attach a scan of both sides of your Shared Access Card.

The Training to Increase Level of Cyber-infection has been discontinued throughout the year, and the Ministry of Defense has reduced extra training and reduced additional jobs. In addition, work of Information Security Officer has stopped. Instead, the Ministry of Defense now requires all uniforms and civilian members to provide an updated personal information to the newly established central isolation of the Pentagon Network Security.
